The link between disability benefit assessments, mental health problems and increases in suicide rates could be made explicit in a new plan announced in medical journal The Lancet.

I think that capturing this data at source is a great idea but it needs to be available to researchers (with patient consent) in order that trends can be identified. Only then can qualitative (anecdotal) data be consolidated in a quantitative (statistical) way. This will give it much needed credibility when used to inform policy development.
